Pediatric cardiology is that branch of medicine that deals with the treatment heart diseases that children are born with (congenital heart diseases) or the heart ailments that develop soon after birth and require surgery. This includes structural defects, rhythm-related problems of heart, as well as the metabolic and rheumatic diseases that occur along with congenital heart diseases.

The pediatric cardiology department of any hospital offers all non-invasive diagnostic and pediatric cardiac interventional services It includes diagnostic modalities such as transthoracic, fetal echocardiography and transesophageal echocardiography. as well as facilities like cardiac catheterization, magnetic resonance and computed tomography angiography. In a pediatric cardiology set-up, cardiovascular surgeons, cardiologists, anesthesiologists and other specialists work together to ensure high quality pre- and post-operative treatment services to children suffering from the above cardio-vascular diseases.

Several cardio-thoracic surgeries such as balloon atrial septostomy, patent ductus arteriosus, atrial septal defect, angioplasty that fall within the purview of pediatric cardiology are briefly described below –

Balloon atrial septostomy

Some newborn children or infants are born with a misplacement of blood vessels carrying pure and impure blood and as a result impure deoxygenated blood is supplied to the body. Balloon atrial septostomy also known as ‘Rashkind technique’ is used to enlarge an opening between the two upper chambers of the heart, so that more oxygenated blood enters the right side of the heart and can be pumped to the entire body.

Patent ductus arteriosus:

Ductus arteriosus, a blood vessel between the aorta and the pulmonary artery (major blood vessels in the heart) closes soon after birth under normal conditions. However, in some infants it remains open causing mixing of oxygenated and unoxygenated blood. This condition is called ‘Patent ductus arteriosus’. This can be treated by surgically closing the ductus arteriosus.

In conditions involving narrowing or blockage of arteries, a process called angioplasty is used to expand the narrowed blood vessels using techniques like balloon dilation.

Valvuloplasty is a procedure to open a narrow or abnormally obstructed heart valve by means of insertion and inflation of a small balloon. This process is also known as a balloon valvotomy or valve dilation.

Atrial Septal Defect (ASD) and Ventricular Septal Defect (VSD):

When the wall separating the two upper chambers of the heart does not close completely, it gives rise to ASD. Likewise, VSD results when the wall separating the two ventricles does not form completely. Both of these defects develop at the time of birth and need to be treated surgically. For the treatment of ASD, an ASD device is placed into the heart through tubes (catheters), by making a tiny incision in the groin area. VSD is also treated by cardiac catheterization.

Tetralogy of Fallot:

This congenital heart disease is a combination of four abnormalities and results insufficient amount of oxygen in the blood. This condition needs to be treated with open heart surgery during first year of life.  The ideal time for surgery is decided based on the baby’s size and symptoms.

A number of heart surgeries, other than those mentioned above are also performed under the domain of Pediatric cardiology.
